Information for referring physicians

Referring Physicians may fax patient referrals to our office at (855) 339-1337.  Please include the patient demographic information, relevant clinical records, referring provider's name and fax number.  Although we accept most major insurance carrier plans, please check with the insurance carrier to verify our network status for your patient.  

Once the patient has been seen, a visit summary will be faxed to you.  Please contact us if you do not receive the visit summary for a patient you have referred to our office.  Most patients are seen within four (4) weeks of the referral request, but appointments may vary as they are coordinated by our office directly with the patient.
