Surgery paperwork must be completed and signed in our office.  Copies of documents will be provided to you or will be available in your patient portal account.  

A signed and dated release is required in order for the release of medical records.

Payment of your copay and any estimated portion owed will be due at the time of service.  Payment in full for services is expected within 30 days of incurred date, regardless of insurance processing status.
